Hello Lopezians – the Lopez Island Hospital District (LIHD) is excited to share that our 2021 Community Survey is now open!
The survey is a very important part of the LIHD’s Strategic Planning process that began in April 2021 and will run through the end of the year. Your feedback will help us understand what you see as the community’s health care needs in the years ahead. The survey results will be incorporated into the LIHD’s actual Strategic Plan document, which will be finalized in early 2022. The Plan will serve as a guide to direct the LIHD’s future activities, define the opportunities and challenges facing our community, and define how we will measure success.
The survey is available in English and Spanish, and you are able to take the survey in one of the three methods described below.
- Take the Online Survey in either English or Spanish. To access the Community Survey in English click HERE. To access the Community Survey in Spanish click HERE.
- Complete a Paper Survey – starting Wednesday paper copies will be distributed via the Lopez Island Family Resource Center and Senior Center’s Meals on Wheels deliveries. Hard copies will also available at the UW Medicine Lopez Island Clinic and Lopez Island Pharmacy. Completed paper surveys can be returned to one of three locations: (1) Drop in the mail slot at the Chamber of Commerce office; (2) in a box marked for the LIHD in the entry of the Lopez Library; (3) or contact Superintendent Presson to arrange to drop off at the District Office located in the Blossom building.
- Take a Phone Survey – if this is your preferred method to complete the survey contact Superintendent Anne Presson and she will arrange a time for you to speak with the independent survey team.
Our goal is to have the survey open for the next two weeks, and it is targeted to close at the end of this month or first week in October. Be sure to share this update and encourage your friends and family to participate.
