Sports Physical Appointments Available at UW Medicine at Lopez
UW Medicine at Lopez Island enjoys caring for student-athletes and is offering sports physicals on April 12, July 15, and August 8. Call (360) 468-2245 and press #2, then press #1 to connect to the clinic and schedule your child’s sports physical today!
If your child has completed a well-child visit within the past 12 months and needs a preparticipation sports history form to participate in athletics, please complete a preparticipation physical evaluation health history form and submit it to your child’s primary care provider for review and signature.
