Start an on-demand video visit or E-Visit questionnaire.

Call the clinic at 360-488-2245:

Press 1 to speak with a UW Medicine centralized scheduler.

Press 2 to be connected with Clinic reception. If Clinic staff are unable to take your call, you may be redirected to centralized scheduling, where you can request a message be sent directly to the Lopez Clinic.

Press 3 for anything else.

For medical emergencies, call 911.
