Accessing Care at UW Medicine Lopez Island Clinic

For all members of the community who use the Clinic for your care, please make sure you have a completed Release of Information form on file. This form will list ALL of your caregivers and allows the Clinic to speak to whoever is caring for you at home. The form can be picked up from a folder on the bench outside of the Clinic entrance and returned via fax or mail.
For anyone not normally receiving care at the Clinic, it’s also suggested you register yourself and family members and complete a Release of Information form. In doing so, you will be able to more easily access care in the event you are unable to get off island to visit your regular provider. You do not have to receive all of your care at the Clinic to be able to register. This practice simply enables the Clinic to create a chart with a few key pieces of information to facilitate any unexpected treatement or testing needed during this crisis, if necessary. This information will also be relevant beyond care needed during the COVID-19 crisis. Establishing yourself with our local Clinic will ease the process should you experience an urgent need such as repairing of a cut, a sprain, or a broken bone.
